Salary Certificate:
If you are a PAYE employee you will require your employer to fill out this salary certificate which details your salary, commission and bonuses.
In addition, you will require a P60 and three recent pay slips to verify your earnings.
Accountant Report Form (Limited Company)
Please have your Accountant complete this form if you are self-employed director of a limited company.
Accountant Report Form (Sole Trader / Unlimited Company)
Please have your accountant complete this form if you are self employed, sole trader or in a partnership.
